- After the colophon (fol. 8a) and on the following page and four folios are several notes (samʻ, ijāzah) about reading Kitāb al-Shifā and the names of those who heard it read. Some notes are dated in the 670s/1270s.
- Collation: Paper ; fol. 12 ; modern foliation in pencil using Western numerals.
- Layout: 19 lines per page.
- Description: Paper tinted red ; lacking many iʻjām diacritic marks ; MS in good condition.
- Origin: Perhaps from the XIVth century? Date discrepancy between the author's dates, and the dates from the reading notes.
- Incipit: بسم الله الرحمن الرحيم ... الحديث الاول حدثنا الشيخ ابو محمد
- Colophon: آخرالاربعين الحمد لله رب العالمين وصلى الله على سيدنا محمد واله وصحبه وسلم تسليما کثيرًا
